To receive a copy of your medical or mental health records, please contact Pennock Health Release of Information staff at (269) 948-3367 or (269) 945-1212 ext. 1367. Our business hours are 7:30 a.m. to 5:30 p.m., Monday thru Friday. Our fax number is (269) 945-4252. Specific fees apply according to your request. For your securty and privacy, you will be required to pick up your records in person, show your ID, and sign for them.
